​We offer various medical services that are not covered by the Medical Services Plan of British Columbia (MSP). These services are payable by the patient. 

*All forms must be handed to the receptionist upon check-in.

Doctor’s Sick Note / Absence Confirmation - $40 

Insurance Attending Physician Statement - $173 

Other Forms* - $40  [per page filled] 
 (*Including but not limited to: Insurance, Employer, School) 

Disability Tax Credit Form - $197 

Driver’s Medical Examination (Yellow Strip) - $248
 
Driver’s Medical Examination (Blue Strip) - $173 patient portion  $75 msp portion

Canada Pension Form - $165 [patient’s portion] 

Private Office Visit (No MSP Coverage) - $95 

Transfer of Records to New Doctor (via Tracumet) - $50

We accept Visa, Mastercard, and Interact/Debit. 
*Visa/Mastercard is accepted over the phone.  
*Debit and Visa Debitcards are not accepted via phone. 
 
We do not accept personal cheques.
